When visiting a managed settlement, it tries to reset itself

Every so often when I fast travel to a settlement, it will pop up a message saying that someone is now the leader (always the same person that I assigned as leader when I started the city plan), and ask me if I want to scrap everything. If I say no, and go to the city planners desk, it says there is no city plan being used.

Any clue what would cause settlements to reset like this?
 
Follow up: turns out it only happens to settlements where the leader is from a mod. I've seen it now with leaders from:
  • Recruitable Leaders Conversion-29795
  • Leaders Of The Commonwealth-30495
  • Sim Settlements - Settlers of the Commonweath - City Leaders-29799
It doesn't seem to happen with any unique npcs from the vanilla game, even when they are made into a leader by a mod.
 
I use Settlers of the Commonwealth and haven’t had this issue. Have tried disabling two of these mods and only using one at a time to see if their potentially causing a conflict with each other.

@Ilja or @OwenHo can either of you replicate this?
 
I am not using those mods, but I have test game going on. Problem is that I am still out of town. I can't do any actual testing until Monday.
 
Had the same issue with starlight driven where I have Preston Garvey set as the leader. Didn't check to see if the plan was missing. I do have a leader mod for SS but have had no issues with the other city I got going with Sturges at the helm. I do however also have AFT (Amazing Follower Tweaks) installed but I can't say it's the issues since I had it installed on my last playthrough and haven't had this bug occur. As far as the SS mod goes I have the Three-in-one version installed, the last playthrough I had SS, IR, and RoTC installed separately for the Xbox one.
 
Does the newest patch fix this issue? If so does it require that you pretty much refresh the settlement and reinstall the leader or no?
 
tl;dr
I just had this same bug at Sanctuary. Sturges was the leader and I'm using Leaders of the Commonwealth. I've been using Post-Nuclear Alchemy since launch.

This happened shortly after I donated a ton of steel scrap (268) to advance the city since it was sitting at 100% level 0. I fast traveled to Starlight Drive-in and dealt with attacking supermutants, then fast traveled back and was told that Sturges is the new leader, which then asked to tear down my settlement. Either option basically resets my city's progress. I don't know if it matters, but I'm also using IDEK's Logistics Station to move all surplus materials to Sanctuary's workshop. Doing this also prevents attacks on Sanctuary. I have yet to have this problem in my other settlements and most of them are managed by added NPCs from Settlers of the Commonwealth, but none of them have reached level 1 yet.
 
This is a sign you need to run the Rebuild Local Data tool in the holotape. If you’re on PC, i’d also recommend doing a manual install to make sure all of your files are up to date.

I tried rebuilding local data at Starlight before traveling to Sanctuary, rebuilding at Red Rocket then walking, rebuilding at Sanctuary before the message pops up, rebuilding after canceling the tear down request, allowing the tear down and rebuilding. The prompt that Sturges is the new leader comes up almost immediately. He must've gotten unassigned after I traveled away which effectively resets my progress.
 
I was thinking on this since I had the same issue myself, why don't we just have a feature that records the progress a settlement makes on the city plan and allows us to restore that progress whenever we encounter an issue like this or any other that cause our work to be undone?
 
No, there's no way to restore the progress.

I don't know why this is happening for some users, as there's literally no mechanism in the mod to remove a City Plan, except for the Take Back City option on the desk.

If someone can find steps for me to reproduce this issue, I'll be able to fix it - but so far I've never been able to. Usually when I can't reproduce an issue, it means one of three things.

1. Your scripts are out of date, and you need to do a manual install. This doesn't really apply to Xbox users though.

2. You have a mod that is causing a conflict. Either the mod is editing something in SS, or accidentally included a script it shouldn't have.

3. It requires a very specific set of circumstances to cause, and no one has figured out what those circumstances are. This last one is when a bug is causing it, and unfortunately, I can't just arbitrarily make code changes as I'm likely to break the mod for everyone NOT having this issue. So I really need detailed steps on how to reproduce it before I can start tackling it.

I've looked over the code, and there's literally only one spot where a City Plan can be removed, and it requires pressing the Take Back City button on the CP Desk.
 
Just had this bug happen to me again. Got a minuteman radiant for County Crossing. Left Dunwich Borers to go to CC; quest to clear nearby training yard. Did so, and got the settlement available. Went to Finch Farm to make a Settlement Creation Kit, and got a popup that Abraham Finch is now leader of Sanctuary, which was previously Sturges. Finch was being built with the RotC plan, and was pretty close to going to level 1 from level 0. Rebuilt the local data; no leader now. Assigned Abigail Finch as leader and had to restart the city from ground up (fortunately, it's rebuilding fast).

Went to Sanctuary, and sure enough, Abraham was listed as the city leader there. Rebuilt the data, which didn't remove him, so I just reassigned Sturges.

ETA: When I built the desk at CC, it showed Professor Goodfeels as leader (he was leader in Sunshine Co-op). I hadn't done anything at CC yet but build the desk. I guess when I go to Sunshine now, it's progress will be borked (it was at level 1). <sigh>

ETA #2: Yup, Sunshine is borked; had McReady as leader, even though I just assigned him to CC. Restored Goodfeels, had to rebuild the settlement. I'm beginning to think all my RotC settlements are borked now.

This is happening to me as well with Ada at Oberland Station, It's happened twice in my current save. Both times seem to be at around the time the city would be due for an upgrade so perhaps that is a factor. This time I tried going to the save before the reset to try without Settlers of the Commonwealth but that didn't help. I don't have any other mods which add leaders but Ada is my only leader from a DLC.
 
This is happening to me too. It started happening immediately after "Banished from the Institute" and you can tell it is happening because all current settler leaders are available to be set as leader at a workbench. I wouldn't be surprised if a character being removed from the list of potential leaders triggers it.
 
For some clarity here is what I am fairly certain about this bug:

1. The bug occurs (I think it is the removal of a leader from the list, it may be more mundane though, like creating so many cities or recruiting so many leaders).
2. This damages the leader select menu, so that leaders normally employed and off the list appear on it.
3. Viewing a leaders card removes them temporarily from their assigned desk. Because you must scroll through multiple leaders to reach a desired one, you may inadvertantly remove multiple leaders from their settlements. Because ADA is typically the first on the list she will usually be kicked off.
4. A leader kicked from their desk resets a settlement to level 0, and removes all scrap, defence and supplies from a settlement desk.
5. If there is a second eligable leader character in the settlement, the leader is kicked from the desk but settlement is not reset.
6. You do not know a leader has been kicked from a desk until you move into range of a settlement.
7. Travelling to a settlement, resetting and reassigning a leader does not remove them from the select leader list as usual. They can still viewed on the list again and reset.
 
I noticed a couple of new things that may have something to do with this bug.

1) The settlement where it is occurring for me (Oberland) was at (or over?) the build limit.

2) If I don't tear down the city when prompted after bug reassigns Ada, when going to the city planners desk it says leader N/A and Ada doesn't show up in the select city leader screen when I try to manually assign her.

Is there anything else we can do to help track down the cause of the bug? I was thinking maybe comparing mod lists to try and narrow down any possible conflicts would be helpful but I'm not sure what the best way to collect the data from everyone would be. Perhaps Google Sheets?
 
To fix it, I basically need a definitive means of reproducing it.

So if it's a specific mod, we need to narrow down which one. If it's just SS, then I need exact steps from a clean save to triggering the bug.

I check around the code every time one of you adds new information, and so far I haven't found a particular cause - which leads me to believe there is a mod conflict here.

I would definitely recommend everyone post their plugins list for comparison.

Added mine; the only difference between what I posted and now is I switched to the 3-in-1, and dropped Vanilla Leaders (I wasn't really using them that much anyway).
