0% food and 0% water

Well, the bar is a % of how well you match you needs. Have you taken out a happiness report from within the Sim Settlement holotape?
Yes. The happiness is 0% along with the the food and water. The number for the vanilla Fallout HUD in build mode show the correct amount of food and water. The modded HUD and Sim settlement report show 0% food 0% water. Also one of the screenshots was of a settlement that was working. Two were of specifically Sanctuary, which is not working.
 
Problem is that other settlement was so small. The needs system is very lenient in small settlements, to get you started. So it doesn't tell us much actually.

I've noticed that this problem only occurs in settlements with characters that can't be killed. I just ran into the bug again at a very small settlement. This one had a child in it. I'll keep playing and see if this becomes a pattern.
 
Interesting. If possible, a mod list and load order might be helpful if you can make one.

Make a save away from Santuary (at least 1/5 of the map).
Exit.
Deactivate the mod.
Start the game and load the save. You should get warning of a missing mod. Ignore that.
Travel to Sanctuary. It looks shit. Ignore that.
Travel away again (at least 1/5 of the map).
Make a new save.
Exit.
Start the game. Load the last save. Travel to sanctuary.
(all this is to make the game reset the sanctuary cell and rerun scripts)

What does the HUDmeters say now?
Yes, I have tried disabling it. I've made multiple new games. Sanctuary usually only breaks once Preston and his crew get there and they don't break until I start using Sim settler stuff. If I was to just get vanilla settlers then Sanctuary works just fine. I've also started to examine what exactly the ENPCs are doing. Mama murphy will actually do what she is assigned to, however the other ones don't go to the plot they were assigned to, they just walk around.
 
Normally it's the other way around. Mama Murphy is notoriously bad at holding on to jobs. (Might be drugs, don't tell anyone..)

Normally if you just leave a few plants for the kids to work on and a bed to sleep on, and otherwise ignore them, they don't disturb your settlement much.

I did ~ and then disable on the kid. Didn't fix the problem though. I mean having a child is a problem in itself, but it didn't fix the bigger problem.

If you go into your command mode, click on the anything, then type disable or markfordelete (set pos z 10 for really tricky things) and they'll be deleted. Sanctuary and Nordhogen beach are broken. Everything else so far is fine.
