Patient troubles

I've completed everything in this quest accept speak with Cassandra but I can't get her to interact with anything other than "well hello". Additionally Ellis is stuck on generic interaction and won't progress to sell me anything

Well this is now a much bigger problem as I cannot proceed with recruiting Cassandra because this quest is stuck. Can ANYONE respond to this?

Can you provide more information please? What platform are you using? Are your mods updated to the latest version? How heavily modded is your game - could you post your load order under the spoiler tag? Are all of the Nightingales present, even if not currently working correctly?
