How to HQ bug

So I'm not sure why but I've created multiple games just to try getting it to work but it does not appear to work so,

It works until it tells you to construct the head of facilities room and after it's completed building no new markers to come talk to Mansfields and I've tried reloading and doing all that but just does not work

Do you mean the Facilities Office? Are you playing in Survival or have otherwise set the SS2 wizard to "Hardcore?"

When I chimed in that I was having a similar issue to the OP (which in my case I think is the same issue you're having), @RobinE suggested that Hardcore mode was the issue. I used the SS2 holotape to change to "City Simulator" complexity and was able to build the Facilities Office (aka Mansfield's office).

Note: you can still be in Survival mode. I only asked about that because if you're playing on Survival, when SS2 loads it assumes Hardcore mode, as I recall, but the SS2 complexity is not inherently tied to the FO4 difficulty.

For me it says Mansfield's office not facilities but yes that is the room I'm talking about and my default setting is City simulator I don't use hard mode and I just attempted to do what you did and it's still not working

Ah, sorry it didn't help. As for the office's name, sorry, I forgot that the office gets called Mansfield's Office. I was mostly trying to distinguish it from his personal quarters that become a construction option later.
