So I finally was able to lock down the exact cause of settlers/civilians popping back up at the end of a Vassal assault and turning on everyone. It will be fixed for 4.1.3.
In the meantime, if you run into this, simply put your weapon down and allow your attackers to subdue everyone. Once the quest completion comes up, the settlers are made immortal again (unless you have a mod that makes them otherwise...). So long as you don't engage, they should not die. It might take a few minutes to process, but eventually everything should calm down.
It turns out, that all of the features we've been adding over the last 6 months have caused the capture code to get way too slow, so I'll need to re-evaluate a lot of it, but I can at least prevent the Civilians turning on us going forward.
Next target is the similar issue that occurs for Liberators, where the settlers will pop up and engage instead of remaining captives...
