You can get this early on in a game just after you have done the intro quest and headed outside, especially if you save and reload (or take a break come back to the game).
From my understanding it is due to the fact that as soon as you start a game there are a lot of background scripts running getting everything sorted... the more mods with scripts you have in your load order the more time it needs to get through them all. if you happen to reload some scripts may be dropped which can cause a lot of issues further into the game... SS2 tries to detect if this is the case and you get a warning. (I recall someone stating that if you progress at a standard pace through the pre-war part and exiting vault 111 that SS2 should be loaded - but...

(I recall someone stating that if you progress at a standard pace through the pre-war part and exiting vault 111 that SS2 should be loaded - but hey many of us have a fair few mods more than just SS2 installed).

If you have a huge load order with lots of mods it may be best to restart again...

Also a search for 'Baking a save'. It might be helpful if you are planning on a long play through (and to try and reduce potential issues down track)
this basically means you start with must have mods enabled, let your game load all their scripts, save, exit enable more mods, load, wait save and so on...
That way you give the best possible chance for a stable game that will last a lot longer...
Also helps to never remove mods once started and be aware that adding mods further into the game can also be problematic (especially if its one of those mods that needs to be present at the very start).
