Disease - Incapacitation Mechanics

In my current game 4, I am playing with disease ON, incapacitation ON and death OFF; and all plot classes and types are unlocked/available at the start (this is the BoS afterall). Each settlement has a clinic, but I have not added any hospital plots yet. I have encountered a case of settler incapacitation (the first one I have observed anyway). The wiki states: "When ON ........ until some of the diseases are cured."
  1. Does the wiki mean my settler will be incapacitated until I get the HQ up/running and research that particular disease or is there also an incapacitation duration?
  2. If the settler is incapacitated until research and a cure are performed, and I turn death ON after the fact, will the settler eventually die?

1. Yes, your settler will be incapacitated until a dissease cure is researched, and a vaccine is developed. Early game, settlers get diseases that they can recover from on their own. Clinics help speed their recovery. Or you can obtain a Disease Cure and cure them. These are the diseases that shouldn't kill them if Death were turned ON.
Later game, they will get more severe diseases that require a vaccine.
2. Yes, if you turn Death ON, they will die from the more serious diseases.
 
Can't they eventually recover from any disease without extra help, given time? Or is that only for the 'most basic' ones?
(last I checked, you could still have people die from Disease - assuming the "can be fatal" option was switched on - before you can even build Municipal Type to get the Graveyard unlock, considering Generic Settlers spawn with maybe a 2 in END at best that early on)

They can recover on their own, for sure, as I said, with the less severe diseases. The ones that can be cured from the start with the Disease Cure for sure, I've seen settlers recover on their own without a Clinic or a Disease Cure. Sturges usually is healthy :)

I haven't tested much with Death ON, not since early testing of C2. At that time, I did have settlers die in the early game, but if I remember correctly, they weren't supposed to with the first outbreak.
Can they die from the early ones? Maybe from the ones that affect Endurance? I published a table of diseases on the Wiki recently on the Diseases page, although I'm not certain all of the dieases listed were implemented, because I haven't seen all of them in game yet, only the ones that are on that terminal in HQ. Honestly, not sure what the calculations are for determining if the settler dies or recovers.

This game, I've been playing with Incapacitation ON, and after completing C2, had an outbreak occur in one of my settlements, but didn't catch which one. I'd gone to investigate why my HQ supplies weren't coming in. All of my settlers were in their beds. The place literally shut down - no food production, no industry, no municipal services, no defense. Happiness had fallen rapidly too. My poor settlers. Makes for more interesting game play.
 
Can they die from the early ones?
From that wiki page:
When ON, settlers whose sickness level exceeds their Endurance have a chance to die each day. The higher the sickness level over Endurance, the greater the chance of death.
Now considering every disease has a "sickness level" of at least 1, and one of the "early" ones is -1 END, and people can have multiple diseases at a time...
 
Ya, so the ones that affect Endurance (should have known that info was there somewhere! :blush I guess not all of the wiki data is filed safely away in my brain)

I still think Diseases start up too early in game. I blame Lily and friends.
 
In addition to your comments, my game answered my next set of questions, that is - after 2 game days, I returned to Oberland where I first observed an incapacitated settler and no one was incapacitated. And now another question - is the early game/late game boundary before/after the quest Is There A Doctor In the House or a later quest?

Also, it does make for more interesting game play.
 
I'd have to ask if you wanted true specifics, but in my gameplay, likely around Character Level 15- 20 ish? With several settlements that have grown and are producing pollution. Usually, I'm able to start Nightingales, having 30 residential plots and 3 settlements, or will soon unlock the hospital through plot progression unlocking.
That point is where I start to see the message "This disease does not have a known cure", when I try to cure my settlers.
The Nightingale quest "Calling ER" has folks with significant disease

My experience with disease has been similar to yours and with my "head canon" playstyle, disease is another, fully appreciated, relevant/interesting game feature from Kinggath et al. So my question regarding early game/late game boundary was more from curiosity, so "true specifics" are not needed. Thanks.
 
I have to say that I just switch off disease; I find it just a tiresome grind without any enjoyable or interesting challenges.
 
Synths are not affected by disease, so another possibility is to play as the Institute and populate your settlements with synths. In addition, they do not need/nor automatically build residential plots. The following Nexus mods worked for me in my recent Institute/synth game:

Or railroad for that matter. Though using only synths is a huge gameplay change to get rid of something like diseases. I kinda wish it was easier to deal with. Like I have a hospital in vault 88 and still had to manually cure everyone. If there even was a cure at that point.
 
While waiting for KG et al to tackle some of the pre 2.2.0 HQ issues, decided to try the Institute for the second time in six years. In addition to no diseases, I found (for my gameplay style) two other advantages to playing the Institute with synths:
  1. Robot/synth happiness - can be set at 100% (Workshop Framework>Settlement Gameplay>Settlement Functionality>Default Robot Happiness Level)
  2. Institute teleportation - teleporting to the Institute and then a map location is about 60 times faster than map fast travel
The only "downside" to crafting the synths is - you can not change the voice and the synths speak in typically settler style.
