Some settlements won't upgrade to level 1, even if they've been at 100% to next level, 100% scrap collected, happiness above 70, and reached the minimum time requirement (around 10 in-game days for the first upgrade, right?). I'm talking about Sanctuary Hills and Tenpine's Bluff, mainly. I have the “leader required” setting set to off, so I don’t have any leaders at my settlements as they seemingly aren’t required with that setting. But maybe that could have something to do with it (?)
Also, after establishing a settlement and placing down the city planner's desk, the settlers place a few plots, upgrade them, and then stop. It seemed normal at first, but I eventually tried refreshing some cities out of curiosity, and in all of them, after refreshing, settlers start rapidly building and upgrading entirely new plots, but not upgrading. It's as if they build for a little while, then stop, and need me to refresh the city to make any more progress.
