I've actually had some issues with this since the 4.1.0 release. I have auto-assign roles on. So, if I assign a warrior to an R/W/E plot they will convert to a worker. If I wait till they are assigned to a plot and then change them to a warrior they stay assigned to the plot, stay a warrior, but...
I'm experiencing this bug as well. All recruits assigned to plots become workers, doesn't matter if it's to a martial plot or recreational, they all become workers.

I have had this issue too, but only when there was a Molotov burning when the last settler/guard went down. The settlement flips to vassal and if one of the settlers gets burned by that Molotov when they start popping back up the settlers will turn hostile. Not sure if your situation was...
