Don't fire the flare. Just POINT your flare gun at the vault door as if it had a scope and then give it a moment. Once you get the notice it is Marked, then fire the flare gun. Should be green.

Another thing that has worked, though whether it is relevant or this particular case, was to assault another random location. I've cleared more than a few markers (usually quest related) by that method! lol
I wonder if they spawned under the ground. In my game the enemy would sometimes spawn a significant distance from the location. Sometimes they were vanilla enemies who happened to sorta kinda be nearby and needed killing too.
